whatever is required based on the personal needs and assessment. Copy
the page(s) and check the appropriate "box(es)" on the page(s).
Use the space provided to give detailed instruction regarding frequency,
intensity, duration as well as repetitions, sets, times per day/week and
length of time to hold the position

Step 3: Re-inforce the program by using the Journal to
give detailed descriptions of daily activities, as well as the Exercise

set dates and times for exercise and activity. Use the Prescription

program. Have this page on the fridge or in a daytimer for constant
reminder. Set short and long term goals for the health and fitness
